Death due to illness

What should I do?

Needed Documents

  1. Claim Request Form Л3-53 – an original with all particulars
  2. Death Certificate transcript – a copy
  3. Death Notice – a copy
  4. All medical documents, associated with registration and treatment of the illness, which caused the Insuree’s death - outpatient cards, tests, discharge summaries, illness history, autopsy report, etc.
  5. Legal Heirs’ Certificate when those are indicated as eligible – a copy
  6. An official document, containing the bank account of each and every heir, if it is not indicated in the Claim Request Form - a copy. For sums over BGN 10,000, the submission of an official document certifying the IBAN number of a bank account is mandatory.
  7. Upon individual and family insurance: Insurance Policy - the original or a Declaration Statement on a Lost Insurance Policy
  8. Upon group insurance at the Employer’s expense - Company certificate from the Employer that the person has been insured as at the date of the event – an original
  9. Upon group insurance at the Insurees’ expense – a copy of the insurance policy and a list of Insurees’ names

    In case the sums will not be received by the eligible person:

    Notarized power of attorney from the entitled person, if the sums will be received by another person. The power of attorney must contain a statement (text) that the eligible person has been notified that he is entitled to receive the sums in person.
