All documents should have ‘true to the original’ attestation, a signature of company’s representative and bear the company’s stamp.

How to become an insurance consultant with DZI?

* If you are a natural person, you need the following documents:

A photocopy of a diploma for a most recently completed stage of education (secondary education as a minimum), with an autographically written „True to the original” inscription and bearing the person’s signature

* In the case of a legal entity, you will need the following documents:

Certificate of Conviction (of the persons, who manage and represent the insurance agent, as well as of the insurance agent’s employees, immediately involved in insurance intermediation activity)

Copy of a diploma for a completed highest level of education (minimum secondary education), of the persons, who manage and represent the insurance agent, as well as of the insurance agent’s employees, immediately involved in insurance intermediation activity, attested with a handwritten „True to the original” inscription and a signature of the respective person
