For reimbursement of expenses for dental (stomatological) care it is necessary to fill out the forms, listed below, as well as provide a set of documents (also listed below). These documents should be provided at an office of DZI or sent to the insurance intermediary, with whom the insurance policy has been concluded. The Insurer has the right to require submission of other documents as well.
