What should I do?

Needed Documents

A filled in Claim Payout Form (for natural persons) as per the Insurer’s template – an original with all particulars
Outpatient card, issued by the attending physician and containing a prescribed auxiliary aid – a copy
A detailed invoice in the Insuree’s name (reflecting the prescribed auxiliary aid by type and price) - an original
An official document, containing the Insuree’s bank account, if it has not been indicated in the Claim Payout Form
